How to refer
- Reach out however is easiest. Call us at (419) 529-9494, email office@molnarleeoralsurgery.com, or fax to (419) 529-9391.
- Send the referral and imaging. See the checklist below — records sent at the time of referral help us schedule your patient without delays.
- We handle the rest. Our front desk schedules the patient, requests anything missing, and keeps your office informed through treatment and follow-up.
If your patient calls us directly, no problem — we'll contact your office to request the X-ray and referral.
What to include with a referral
- Panoramic X-ray — or CBCT if available
- Written referral with the reason for referral, tooth numbers, and any relevant notes
- Any additional films you have (periapicals)
- Intraoral photos for pathology and biopsy cases
