Everything you need before your first appointment and on the day of surgery — so there are no surprises.
Before your first visit
When you call to schedule
Scheduling goes fastest when you have these ready:
- The patient's full legal name and date of birth
- The reason for the visit, and which tooth or teeth if known
- Your referring dentist's name and practice
- Your dental and medical insurance cards — we collect both when you schedule
We'll contact your dentist's office directly to request your X-rays and referral — you don't need to carry records around.
Registration
Please complete patient registration before your appointment, and be prepared to provide a comprehensive medical and surgical history.
What to bring
- Your surgical referral slip and any X-rays, if applicable
- A list of your current medications
- Your medical or dental insurance information
X-rays from your dentist or physician can be forwarded to our office; additional imaging may be required. All patients under 18 must be accompanied by a parent or guardian.
If you're scheduled for surgery
- A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia.
- No food or drink after midnight the night before surgery with IV anesthesia.
- A responsible adult driver must take you home.
- Tell our office about any medical conditions — such as diabetes, high blood pressure, artificial heart valves or joints, or a history of rheumatic fever — and any medications you take, including heart medications, bone medications, or anticoagulants.

Insurance & payment options
We are not an in-network provider with insurance plans, but we are happy to assist in submitting your treatment to your insurance provider.
