Most patients are referred by their dentist or physician, but you can also call us directly at (419) 529-9494. Either way, our front desk contacts your dentist's office to request your X-rays and written referral — you don't need to track down records yourself.
Please remember to bring your surgical referral, any relevant X-rays, and a current medication list, along with your medical and dental insurance cards. We also ask that you finish the patient registration forms — which our team will send once your appointment is scheduled — prior to your arrival.
While Dr. Lee is an out-of-network provider, we do everything we can to make the financial side of your care seamless. We will handle the heavy lifting by submitting all claims directly to your insurance provider on your behalf. Before we begin any treatment, we will sit down with you and map out a clear, upfront breakdown of your expected out-of-pocket costs so you can make an informed decision without any financial surprises.
We offer a range of anesthesia options, from local anesthesia to IV sedation, and Dr. Lee will recommend the safest option for you at your consultation. A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia.
If you are having IV anesthesia, have no food or drink after midnight the night before your surgery. You will also need a responsible adult driver to take you home.
No. All patients under 18 must be accompanied by a parent or guardian.
